When you are ready to submit all documents, come back to this same website and click next through the modules without watching. You do not have to fill out the questionnaire again.

Click on the arrow right next to the documents below and download, save them, then type in responses. Remember to save after filling out and submit completed forms.

Application Saving Instructions

When you are saving your documents it should be as follows: (make sure all documents are easily readable , all have your name on it, and they are right side up. (not side ways or upside down). Make sure TEAS results are the correct report as shown in the information session module.

LVN Application and Criminal History Packet: save to your computer in WORD format and type in responses. Do not hand write.
Transcripts: Save your Navarro College Unofficial Transcript in the appropriate tab. Any other college transcripts (if those classes are NOT on your Navarro College Unofficial Transcript) are submitted in a different tab called "Other College Transcripts". (You will see the different tabs when you click on the "Submit LVN Application Packet" button on this page).
Immunizations: Scan all as ONE FILE (will more than likely be PNG, PDG, or PDF format). (Make sure your name and facility are on this). Do NOT COPY AND PASTE THESE ON A WORD DOCUMENT!) NO PICTURES OF THE DOCUMENTS PLEASE. ALWAYS SCAN AND UPLOAD RIGHT SIDE UP.
High school transcript: You DO NOT need to submit unless you are currently a high school student or you were a high school student in the previous year and had a 3.5 GPA or greater, or you want to claim points for the HOSA course if you took that in High School in any previous year.
TEAS Results submission: Your TEAS results must have your name, date you took the exam, number of attempts, and days since last attempt. If you can take your TEAS exam at a Navarro College, please ask them to print the result page for you when you are done. Make sure the result page they give you looks like the example in the LVN Information Session Packet Document in Module #1, and it is also in the video modules.! If you do NOT have your results, you can call ATI at 1-800-667-7531
Health Care Experience or Certification: Remember in order to get points for Health Care certification you must submit certification with expiration date if applicable. If you do not have a certification, but you do have health care experience, please submit a letter from your employer saying dates of employment and what your overall job responsibility was in the position. Please scan multiple pages of a document as ONE file.
If you are claiming points for a degree, make sure that your degree is on the Navarro College Unofficial transcript or you must submit a degree certificate. Make sure you document it on the application as well to ensure you receive points.
No links to transcripts or other documents are accepted. Only scanned or saved documents should be uploaded.
